ABOUT THE OF-169

-Birth certificate

If born in the Netherlands obtain the International version of the birth certificate (doc.A)

-Police certificate

You can NOT obtain the Dutch Police certificate. The Consulate will obtain it for you but can ONLY do so
after you have submitted the OF-230 part 1 !!!!!   (Now the DS-230 part 1)

-Military records

If you served in the Dutch Army you can write to;

TIP If you request a statement in the English language they will do so.

-Photographs    (In contrary to the standard OF-169 instructions you need 4 pictures for package 3)

Be aware that Dutch passport-pictures are not the same as the pictures you have to provide.
LOOK TO THE LEFT. !!!!!!
Get at minimum 5 pictures. You need 1 for package 1 and you need 4 to accomplish packet 3. !!!!!!
Print the picture instructions and take it with you to a photographer. Our photographer made
it with a standard passport-picture camera (Polaroid) he only moved the light to the other side
and moved closer with the camera.

ABOUT THE AFFIDAVIT OF SUPPORT I-864
In addition to the I-864 you need;  (general guideline read the instructions..!!!)
Your W-2 and 1099 forms from the most recent 3 tax years.
A few recent pay stubs to prove you are still employed.
Evidence of current employment or self-employment, such as a recent (within a month prior
to the signature date of the I-864) pay statement or a statement from your employer on
business stationary, showing beginning date of employment, type of work performed and salary
or wages paid.
DO NOT SIGN THE FORM !!!!  This needs to be done in front of the Consular officer unless
it is notarized.
There are a lot of possibilities like using assets, you need to read ALL the instructions.

ABOUT THE VACCINATION REQUIREMENTS
You need some vaccinations prior to the issuance of an immigrant visa.
I went to my home-doctor and the health insurance company did pay the bill.
Ask for a statement you had the required shots and bring it with you for the medical exam.
Otherwise the assigned doctor at the medical exam will give the vaccinations,
you have to pay him cash.
